Case Management
REM West Virginia’s Case Management program provides assessment, service planning and coordination, advocacy services, and assistance with accessing community resources and funding for the individuals and families we support.
Each individual referred to one of our programs receives a formal assessment. This assessment is used to develop an individualized plan, which includes the individual’s lifestyle preferences within a full range of residential, work, social and other community-based services and supports.
We believe that providing quality services and supports is our primary responsibility. That is why REM West Virginia ensures that each individual is getting the quality services they need while making personal choices with regards to their life, inclusion within the community and any activities related to their development and growth.
Our staff continually monitors and evaluates each person’s plan and makes adjustments as the individual’s needs and preferences change over time. Service Coordinators provide:
- Linkage and referral
- Advocacy
- Service planning
- Assessment
- Crisis response planning
- Service planning evaluation
REM West Virginia’s comprehensive approach to Case Management brings together individuals, families and providers for the benefit of the individuals we support. The individualized attention each person receives from his or her Case Manager and their holistic approach to service coordination helps each individual to achieve their own personal goals for living and working in the community.
REM West Virginia also offers Case Management services for emergency situations.
